He'd cut off his own "on" arm...I know 'cause something very similar to this happened in one of my campaigns a while back

depends on your 2nd e roll on the fumble table. in 3e there is no fumble. thou a 1 is auto-matic miss. n considering weapons, class, n random events. a 6th lvl fighter can have a +15 to-hit. you need a 1.

There was a certain theif I remember. A REAl clever schemer. He had the Audacity to ask a dragon if was willing to sell some eggs! He was, or course, Crisped rather thorougly. I made sure the Dragons scattered the ashes and such. The rest of the party had enough warning to get the heck out of there. A rare moment it was when he realized what he had done too. A year of building up a theif literally went up in flames.
